8. Whenever a safety representative uses an apparatus or instrument necessary for exercising his functions, he shall draft a report addressed to the health and safety committee or, if there is no committee, to the workers or their accredited association and to the employer, containing the following information:(1) the time and date of the measurement and the place where it was taken;
(2) the instruments or apparatus used;
(3) the duration of the observation;
(4) the results observed; and
(5) a description of the environmental conditions prevailing at the work place at the time of the observation specifying, where applicable, to what extent the conditions are exceptional.
The measurement results must be entered in the report immediately after they have been observed.
The report must be drafted as soon as possible, and signed by the safety representative who shall enter therein the time and date of its completion.
